The first time you visit a doctor's office you are often given a clip board filled with paperwork you must first fill out before seeing the doctor. Not at Willowbrook Cardiovascular Associates!

At Willowbrook Cardiovascular Associates we'd like you to be able to fill out our forms in the comfort of your own home, without feeling rushed.

To save you time and last-minute stress, please download the documents listed to the right, fill them out at your convenience, and then bring the forms with you to clinic.

Having a copy of your previous records helps us treat you without, many times, having to repeat studies or labs.

If you have been: 1) recently discharged from the hospital or other medical facility, 2) seen another cardiovascular health care provider, or 3) seen another health care provider for cardiovascular related tests (lab draws, stress tests) - please bring a copy of the records (results) with you to clinic.

If you do not already have a copy - you can call the facility where the test(s) were performed, and speak to the "medical records" deparment. They will give you instructions on how to obtain your records. Most times, if possible, this is best accomplished in person.

If possible, please bring a copy of your insurance information.
